Where it came from
The flag is based on the flag of the All-India Muslim League, the party that campaigned for Pakistan. That flag was green with a white crescent and star, and was itself inspired by Ottoman flags.
Who designed the national flag is not fully settled. Wikipedia and a 2016 academic source credit a team led by Syed Amir-uddin Kedwaii, while the current official government page credits the country's founder, Muhammad Ali Jinnah. Tailors in Delhi are said to have sewn the first flags.
History
Before independence, Lord Mountbatten suggested putting the British Union Jack in the corner, but Jinnah refused. The flag was adopted on 11 August 1947 and has not changed since. It has been the flag of the Islamic Republic of Pakistan since 1956.
